What To Expect At Your Primary Care Provider Appointment

At Valley Professionals Community Health Center, a visit with a Primary Care Provider may be scheduled with a Nurse Practitioner, Physician, or Physician Assistant.  

The purpose of your visit may be: 

  • An initial visit to meet a new provider 
  • A routine visit to monitor a chronic health condition such as hypertension or diabetes  
  • A routine wellness visit to ensure all age appropriate preventative measures are up-to-date 
  • An acute or same day visit to address a simple illness or injury  

Regardless of the reason for your visit, you can expect some basic processes: 

Registration: The front office staff will greet you and ensure we have your most current information such as name, address, insurance, etc.  If you have a family member or friend, you would like to have access to your health information and speak to the medical staff, notify the front office staff to include them on your HIPAA list.  This protects your medical records and information.  

Nursing staff:  Our friendly nursing staff will obtain and verify multiple pieces of information such as vital signs, recent surgeries or hospitalizations, list of specialists (such as cardiology), and more. An IMPORTANT item for both patient and provider is an up-to-date list of your medications and allergies. Bringing a list of medications or actual bottles with you will ensure we maintain an accurate medication list. Be certain to include any over the counter and alternative medications.  

Provider:  During the time with your Healthcare Provider, it is important we address issues most concerning to you; bringing a list of questions or concerns to discuss will help this goal. While time may prevent you from discussing all your concerns, you and your provider can prioritize and plan for future visits.   

An appointment with a Primary Care Provider can be anxiety producing for some people. Knowing what to expect at your visit, and being prepared, can allay fears; and make your visit a positive experience.
