Community Health Workers Join VPCHC to Assist with Insurance Enrollment

Both the Affordable Care Act (ACA) and Insurance Marketplace are going into effect on October 1, 2013. Vermillion-Parke Community Health Center (VPCHC) is pleased to announce the addition of Community Health Workers (CHW) to its staff, who are both federally and state certified, to assist individuals with insurance enrollment in the ACA or marketplace.

Both the Affordable Care Act (ACA) and Insurance Marketplace are going into effect on October 1, 2013. Vermillion-Parke Community Health Center (VPCHC) is pleased to announce the addition of Community Health Workers (CHW) to its staff, who are both federally and state certified, to assist individuals with insurance enrollment in the ACA or marketplace.

If you are uninsured or believe you may qualify for Medicaid, open enrollment for the Insurance Marketplace will take place from October 1, 2013 through March 31, 2014. To receive insurance by January 1, 2014, an application must be filled out no later than December 15, 2013. A single online application will inform you of eligibility in Medicaid or for one of the insurance healthcare plans offered through the Marketplace. The role of the CHWs will consist of linking the healthcare field to the community they serve. The responsibilities of our CHWs will include:Assist individuals/families in navigating through the Insurance MarketplaceConnect patients to local resources in the communityCounsel patients on preventive health care behaviorsReduce unnecessary hospital readmissionsCompliment the Patient Center Medical Home Model

The CHWs will be in the community in many locations such as our VPCHC’s three health centers in Clinton, Cayuga, and Bloomingdale, as well as the Mobile Health Center, county courthouses, and local libraries. Additional places may include: Union Hospital (Clinton), Rockville Learning Center, local food banks, sporting activities, and many other events in and around the community.
