Local Community Health Workers Help with Insurance Enrollment

Vermillion-Parke Community Health Center is pleased to announce the addition of Community Health Workers (CHW) to its staff. The role of a CHW involves linking health care services to the communities they serve.
The responsibilities of a CHW include assisting individuals and families in navigating through the new Insurance Marketplace, connecting patients to local resources in their community, counseling patients on preventive health care behaviors, reducing unnecessary hospital readmissions, and complimenting the Patient Centered Medical Home model.
The CHWs will be at various community locations such as VPCHC’s three health centers in Clinton, Cayuga and Bloomingdale, as well as the Mobile Health Center, county courthouses, and local libraries. Additional places may include Union Hospital Clinton, Rockville Learning Center, local food banks and many other events around the community.

VPCHC is a Federally Qualified Health Center (FQHC) dedicated to providing comprehensive primary health care and related services for ALL individuals and families. VPCHC provides primary and preventive health care services, along with behavioral health services to the citizens of Vermillion and Parke counties, and surrounding communities.

VPCHC was awarded a $50,000 challenge grant through the Wabash Valley Community Foundation that will help to fund bring primary, behavioral health and pharmacy services to West Terre Haute.
