Sliding Fee Scale

If you are worried about paying your medical bills, we can help you through one of the financial assistance programs at Valley Professionals.

Insurance

Dental includes everything from preventative care to emergency and restorative services.

Medicaid / Medicare

We accept traditional Medicaid, Medicare and most managed care plans.

Sliding Fee Scale

If a patient is uninsured or underinsured, a sliding fee scale is available for patients to receive a discounted rate. Discounts available are based on patient family size and income in accordance with the most recent U.S. Department of Health & Human Services Federal Poverty Guidelines.

The sliding fee scale is based upon the current income of all family members, and the number of people in the household. An individual who is unemployed, has no income and is being supported by others must provide documentation showing that no federal income taxes have been filed, and the person supporting the patient must provide written and signed documentation that they are providing food and shelter for the patient. In order to qualify for the sliding fee scale at VPCHC, patients and their families must have a combined income that falls within the U.S. Federal Poverty Guidelines.

We Accept Most Major Commercial Insurance

Health insurance can be overwhelming, but let’s simplify it! Insurance includes the medical coverage for areas such as health care, dental or vision. Valley Professionals accepts most insurance carrier types, including Anthem, United Healthcare, AMBetter Care Source, and more.

Medicaid & Medicare

Valley Professionals accepts Medicaid and Medicare. We accept all four Medicaid managed care plans and traditional Medicaid, including Healthy Indiana Plan (HIP), Hoosier Healthwise, Hoosier Care Connect. These plans may include Anthem, CareSource, MDwise, or Managed Health Services (MHS).

For Medicare, we accept traditional Medicare and your supplement. We also accept most managed care Medicare plans.
